Careplan Care Managers

Navigate Adult and Senior Care with Confidence and Compassion

When aging, health changes, dementia, or changing family needs make care decisions feel overwhelming, Careplan Care Managers are here to help. We provide comprehensive senior care, elderly care, mental health, and life care planning services for adults and older adults throughout the Greater Cleveland Area.

Our experienced care managers help families understand complex healthcare decisions, coordinate medical and home care services, explore senior living options, and develop personalized plans for current and future needs. We also provide specialized dementia and memory care support, family advocacy, recreational care, and caregiver guidance.

Whether your loved one wants to remain safely at home, needs help managing chronic health conditions, is experiencing memory loss, or requires a transition to assisted living or skilled nursing, Careplan provides compassionate, knowledgeable guidance every step of the way.

Meet Our Careplan Team

This may be a challenging season in your life, but you don’t have to navigate it alone. Our experienced team walks alongside you, providing education, advocacy, and personalized guidance to help you find caring solutions for your loved ones.

At Careplan, we believe effective elder care, senior care planning, and mental health care begins with understanding the individual—not simply their medical condition. Our care managers work closely with older adults and their families to identify needs, explore options, coordinate services, and create practical plans that support health, independence, safety, and quality of life.

Bridget Ritossa is a Certified Care Manager and Licensed Social Worker with more than 30 years of experience working with older adults and families. Throughout her career, Bridget developed extensive knowledge and resources in the areas of aging, geriatric care, healthcare navigation, social work, and senior care planning while working within two major hospital systems in Cleveland, including two of the largest healthcare organizations in the country.

Above all, Bridget’s goal was to create a company that gives adults and their families the resources, education, advocacy, and knowledge they need to live well at every stage of life.

That vision remains at the heart of Careplan Care Managers. Every individualized life care plan is designed around the unique needs, goals, preferences, and circumstances of the person receiving care.

Is Caring for an Aging Parent Becoming Overwhelming?

You’re not alone. Caring for an aging parent or loved one can quickly become overwhelming. Elder care coordination, senior care planning, medical navigation, mental health concerns, dementia care, home care, and finding trusted senior living options can feel like a full-time job—especially when you’re balancing work, family, and other responsibilities.

Careplan Care Managers provide the professional guidance families need to navigate these challenges with greater confidence. Whether you’re managing dementia or Alzheimer’s care, coordinating medical appointments, exploring long-term care options, developing a life care plan, supporting mental and emotional well-being, or living far away from your loved one, our certified care managers can help.

We work with families to identify needs, develop individualized care plans, coordinate resources, advocate for loved ones, and make informed decisions about their next steps.

Testimonial

Before working with Careplan, I felt lost after leaving a treatment program. They helped me organize my appointments, reminded me to follow through with my doctor, and encouraged me when I wanted to give up. Having someone who understands mental health but also helps with the practical day-to-day parts has made all the difference. I’m more stable now — and more hopeful.
