Northeast Ohio geriatric patients meet with Careplan Geriatric Care Managers for consultation. Also, we will meet at home, at the hospital, or any off-site location throughout Northeast Ohio. Careplan serves counties such as Cuyahoga, Lake, Geauga and Ashtabula Counties.
